This Web site provides students, faculty, staff, alumni, and guests with a Penn State User ID with a Web form for reporting acts of hate or intolerance that occur at Penn State. Students may use the form to report any act of intolerance they experience themselves or observe directed against anyone else. However, all other groups should only use the form to report acts they observe that are directed against students. If no student is involved, other groups should contact the Affirmative Action Office at 814-863-0471.
In addition to this form, other means exist for reporting acts of intolerance. Here at Penn State DuBois, you can call Rebecca Pennington, the Director of Student Affairs, at 814-375-4766. If you or someone you know is in an emergency situation that does or could require medical, psychological or police services, please call 911 or Penn State Police Officer Cody Haag at 814-375-4734.
Statement of Confidentiality
Please do not use the Report Hate Web Form if you are currently in a situation where immediate medical, police, psychological, or other emergency services are needed. In case of an emergency, call 911 to get the appropriate services.
Responses submitted through the form are encrypted and sent to a secure server to insure the confidentiality of users.
Only Penn State constituents may use the system. The system requires users to logon with a valid Penn State Access Account user ID and password. Penn State staff who read reports generated by the system have no access to user ID information so reports generated are completely anonymous to these staff. However, should a report indicate or imply that the user or someone else is in danger, the report will be turned over to appropriate Penn State computer and security personnel, who do have access to the user ID of each report. User IDs can be used to identify those who submit reports. Reports could also be turned over if deemed appropriate under other circumstances. Also, reports are subject to subpoena under certain circumstances.
